(GPRA) FY 2009 Forms and Instructions

The GPRA form is part of the Weed and Seed Grant Application Kit and is due annually. Sites that have reached their fifth and final year of funding are still required to submit the GPRA form for activities that take place within that year.

NOTE: Do not use the online form to submit corrections or updates. Please fax (202-842-9329) or send an email to your state's assigned staff member found on the Contacts listing.

Those utilizing the Adobe Reader program will not be able to save a completed or partially completed form via Adobe's save features. In order to save a partially completed form with data, go to page 6 of the online GPRA form and utilize the "Save Copy via E-mail" button. Your e-mail address on page one must be filled out correctly. Once you have completed your form you should complete the following steps to ensure that you have a copy of your completed GPRA Form:

1. Print the completed form
2. Hit the Submit button on page 6, an e-mail will be sent to the address on page 1 with a copy of your submission attached.
3. Without opening the attachment, save the attachment to your computer, network or storage device.
